When Undertaker's theme have "extra" sound effects in them, they're not actually part of the track. They're separate sound effects played over the song. I also don't get this obsession people have with counting the bells before theme as part of the song, making it a new theme on the counts. It's a separate sound effect.
The current 'Taker theme has numerous titles, just pick whichever you like. I prefer just using "Graveyard Symphony". It's the same song as any of the versions from 1995, just different instrumentation. I don't see Pearl Jam's recent remakes of their classic songs getting different titles just because it's a new version of the song.

I'm not completely sure, but the last time I downloaded the cassette tape recordings, what I heard was his 1991 and his SummerSlam '94 themes, not the Full Metal theme. The SS94 and Full Metal themes sound similar and share pretty much the same pitch and length, but have different instruments and different gongs, as far as I can tell.
Also, even though Graveyard Symphony V3 would make for an appropriate name (V2 being the version from WWF In Your House PSX), when songs have been remade, sometimes they've been given new names. As in 'Taker with "Dark Side" (Vol.3) and "Ministry" (Vol.4) / "Dark Side" (Anthology), Edge with "You Think You Know Me" and "On The Edge", or Kane with "Burned" (Vol.3) and "Out of the Fire" (Vol.5) / "Burned" (Anthology). That's what probably happened with his 27th, when it was renamed "The Darkest Side" in ThemeAddict, Vol.6, and "Rest in Peace" in RAW Greatest Hits/Voices' Legends CD. It's not that I'm trying to force anybody into calling the themes that way or anything, I'm just pointing it out.

All of the themes on Volume 3 had no titles. I don't know where people connect "Burned" to Volume 3.
I don't get why Jim Johnston renames themes. I just use whatever the original title of the composition is.

Well, my apologies for not citing sources for the Vol.3 titles. It seems that they were an invention of the people who listed the themes in other sites... As for renaming matters, we may never know why.

That list to my knowledge is correct, and no, he didn't use the WWF Aggression track.
